Understand what skilled means
Examples include skilled nursing or therapy that meets Medicare’s requirements. Needing help with everyday activities alone is different. Ask the treating team to describe the specific skilled service and why it is needed. Treatment decisions belong with your clinicians.
Check the admission requirements
Original Medicare generally requires a qualifying inpatient hospital stay, daily skilled care, and a Medicare certified facility. Care usually must begin within 30 days of leaving the hospital. Observation time does not meet the inpatient stay requirement. Waivers or different Medicare Advantage arrangements may apply.
Do not build a budget around 100 free days
Original Medicare may cover up to 100 days per benefit period when requirements continue to be met. Cost sharing changes during that period, and coverage can end sooner. Ask for your expected daily responsibility and the date that amount could change. Medicare Advantage costs vary by plan.
Keep clinical progress and coverage together
Ask who communicates progress, who handles authorization when required, and who tells the family if covered care is expected to end. Designate a family contact with appropriate permission. Keep a dated record of conversations so everyone understands the next step.
Prepare for what follows
Before discharge, ask about equipment, follow up care, home support, and written instructions. If you receive a notice ending covered services, read its appeal instructions immediately. A plan comparison later cannot substitute for acting on a current deadline.
