Do I need Part D if I take no medications?
You are not required to enroll. But skipping drug coverage without another creditable source starts a penalty clock that adds about 1% per month to your future premiums, permanently. Healthy is exactly when coverage costs the least.
1% per month, forever
Go 20 months without creditable drug coverage and every future Part D premium costs about 20% extra, for life. The clock starts 63 days after your Initial Enrollment Period ends.
Creditable coverage
Drug coverage from an employer plan, VA benefits, or TRICARE counts as creditable and keeps you penalty-free. Keep the annual notice that proves it.
A low-premium plan
Enrolling in the lowest-premium drug plan in your area keeps the clock at zero and buys the 2026 protection everyone gets: covered prescriptions capped at $2,100 out of pocket for the year.
What happens when you need drugs later
Prescriptions rarely schedule themselves for October. Without coverage, a new medication means paying cash until the next Oct 15 to Dec 7 window, then a penalty-loaded premium after that.
With even a minimal plan, you switch to one that covers your new prescription at the next window with no penalty at all. Every fall we re-run our clients’ drug lists against the coming year’s plans. The enrollment windows, explained.
Related questions
I get my medications through the VA. Do I need Part D? +
VA drug coverage is creditable, so no penalty builds while you have it. Some veterans still add a Part D plan for pharmacy flexibility, but it is a choice, not a requirement.
Is drug coverage included in Medicare Advantage? +
Usually yes. Most Advantage plans bundle drug coverage, which satisfies the requirement automatically. Check before assuming, since a few plans, like some designed for veterans, leave it out.
When can I add Part D later? +
Each fall from Oct 15 to Dec 7 for a January 1 start, or during a Special Enrollment Period if you lose other coverage. The penalty, if you built one, arrives with the new plan.
One question about your coverage now can save years of penalty later.
